Eligibility & application
- Obtain certification from a registered physician for a qualifying serious medical condition.
- Apply through the OMC portal with a passport-quality photo, ID, residency proof, physician certification, and fee or hardship-waiver documentation.
- After approval, download the digital card from the portal. Renewal requires an annual physician appointment and a renewal application.

OMC application fee $50. A financial-hardship waiver may be available; physician evaluation fees are separate.
