Eligibility & application

  1. An eligible Oregon attending provider must complete the Attending Provider Statement after reviewing records and conducting the required examination.
  2. Apply to OMMP online or by mail with a statement signed within 90 days, residency ID, and the applicable fee.
  3. Renewals require a new statement. Oregon Medical Board restrictions on telemedicine-status licensees apply even when a provider advertises online evaluations.

OMMP standard application/renewal fee $200; SNAP $60; OHP $50; SSI $20; armed-forces service $20; $0 for certain veterans with a total disability rating of at least 50%. Green Health Docs evaluation fee published as $200.
