Eligibility & application
- A participating provider supplies an Authorizing Healthcare Practitioner Statement with the required reference and registry IDs.
- Create a CRC patient portal account, select an Alternative Treatment Center, and submit a recent photo and New Jersey residency proof.
- Choose a free digital ID or a paid physical card. The registration term is two years.
Published fees
Digital ID free; physical ID $10; valid two years.
