Eligibility & application
- Obtain a written provider certification and complete the patient application. Include New Hampshire residency proof and the application fee.
- Mail or deliver the packet to the Therapeutic Cannabis Program. Provider signatures are valid for six months.

Published fees
Patient application or renewal $50. Replacement card $10. No caregiver application fee. No hardship reduction.
