Eligibility & application
- Be a Delaware resident age 18 or older, or a pediatric patient with parental consent. Obtain certification from a Delaware-licensed MD, DO, APRN, or PA.
- Patients 65 and older may self-certify but must still submit the patient application.
- Apply online if your practitioner participates in the online system, or use the paper application. Provide Delaware ID and pay for the selected card term.
Published fees
State fee $50 (1 year), $75 (2 years), or $100 (3 years).
