Eligibility & application
- Obtain a physician recommendation for an eligible medical condition. The state Medical Marijuana Identification Card (MMIC) is voluntary.
- For the optional MMIC, apply in person through your county health department with the recommendation, photo ID, and proof of residency.
- The county issues the card; an evaluation provider does not issue the state MMIC.
Published fees
County MMIC fee not to exceed $100; Medi-Cal not to exceed $50; waived for CMSP indigent patients. Leafwell evaluation published at $35.
