Eligibility & application

1. Be an Alabama resident with a qualifying condition. Patients under 19 need a parent or legal guardian registered as caregiver.
2. See an Alabama physician certified for the program in person, in Alabama. Telemedicine is prohibited for certification and recertification.
3. After the physician enters the recommendation, complete the AMCC patient registration and pay the applicable fee. A valid card and an active physician certification are separate requirements.

Published fees

State digital card $40; optional physical card $10; replacement physical card $25. Green Health Docs advertised an initial visit at $200 and follow-ups at $100. Confirm charges before booking.
